AMS 800 is a hydraulic system that imitates the sphincter of the urethra and enables a high degree of urinary continence in incontinent patients.

 

Implantation of the AMS 800 sphincter is considered the gold standard for the treatment of urinary incontinence after radical prostatectomy.

Artificial urethral sphincter surgery is also considered when male urinary incontinence occurs after open, laparoscopic or robotic radical prostatectomy, transurethral resection of the prostate (TURP), photoselective vaporization of the prostate (PVP), holmium laser enucleation of the prostate (HOLEP) and incontinence after prostate radiation therapy.

 

The frequency of incontinence after radical prostatectomy ranges up to 40%. There is no significant difference in the frequency of incontinence after radical prostatectomy performed laparoscopically, robotically, or by open surgery.

 

Implantation of an artificial sphincter in men is never performed at least 6 and sometimes 12 months after the operation that led to incontinence.

In those first 12 months, urinary incontinence may improve on its own without intervention. On the other hand, improvement is considered unlikely after this period.

 

An artificial urethral sphincter is used in the treatment of neurogenic bladder dysfunction with incontinence.

 

In patients whose urinary incontinence is the result of simultaneous incompetence of the sphincter and bladder muscles, sphincter implantation (as urogenital reconstructive surgery for urinary incontinence) can be considered as a very effective solution.

 

Candidates for artificial sphincter implantation must have normal manual dexterity and a healthy bladder.

The evaluation of all potential candidates for artificial sphincter implantation involves:

Detailed medical history and examination

Urinalysis

Urethro-cystoscopy

Urodynamic examination of the urinary tract

 

The artificial sphincter insertion operation is performed under general or spinal anesthesia.

The duration of the operation is about 90 minutes.

The hospital stay is 1 day.

Postoperative dressing is performed for 2 to 3 days.

Control urethroscopy is performed 4 to 5 weeks after the operation, when the artificial sphincter is activated.
