What to expect from penile enlargement surgery?
Depending on the surgical technique, penis girth enhancement is cumulative, meaning it can be repeated several times with each new intervention providing larger and more permanent results.
Penis lengthening is NOT cumulative and can only be performed once.
Permanent penis enlargement is only achieved through PLGA scaffold implantation, dermolipectomy, scrotoplasty, or corporoplasty. These are complex urogenital reconstruction procedures successfully performed by very few genital surgeons. This is why these procedures aren't widely popularized among doctors and aren't easily accessible to men seeking permanent penis enlargement.
Penis enlargement with hyaluronic fillers, pubic liposuction, ligamentolysis, and lipofilling are procedures that don't provide permanent enlargement results but are most accessible to men due to the large number of doctors offering these services.
Due to potential shortening and tightening of internal scar tissue after surgery, a penile extender or vacuum device may sometimes need to be used.
For better healing of surgical wounds, the use of skin care creams and scar prevention is recommended.
Permanent penis girth enlargement with biodegradable PGLA scaffolds
A biodegradable scaffold is a laboratory-created polymer cylinder surgically implanted under the penile skin. The scaffolds are specially prepared before the procedure using platelet-rich plasma (PRP Matrix) and hyaluronic acid. Over 3-4 months, the biodegradable scaffolds gradually get replaced by natural tissue under the penile skin. Post-surgery, seroma accumulation under the penile skin often occurs, requiring additional surgery for removal, which may damage penile skin vascularization. Although results are permanent, due to frequent complications and high material costs, this method isn't justified.
The surgery is performed under general anesthesia.
The procedure lasts about 60 minutes.
Hospital stay is one day post-operation.
Follow-up checks are done at two days.
Abstinence period is 10-12 weeks.
The procedure can be repeated after 12 months if desired. Maximum three repetitions are possible.
Penis lengthening by cutting suspensory ligaments
- Ligamentolysis -
By cutting the penile ligaments, the "hidden length" of the penis can be released. The surgical approach is usually above the penis with V-Y skin plasty for additional aesthetic lengthening. An alternative approach with excellent aesthetic results is through a scrotal incision or circumcision cut to avoid scarring above the penis.
Ligamentolysis can be combined with penis enlargement using the Lipocube Microfat system and scrotoplasty.

Penis lengthening through pubic region liposuction
This procedure is performed together with girth enlargement using the patient's own fat cells. Besides requiring general anesthesia and postoperative recovery, this method rarely causes complications like edema or hematoma. The drawback is possible fat tissue regrowth in the liposuction area.
Penis lengthening by surgical removal of fat and pubic skin
- Dermolipectomy -
This procedure is also called the "Mini tummy tuck" procedure. By removing excess skin and fat tissue (the lower abdomen) covering the penile body, full exposure of the entire penile length is achieved. The pubic skin is repositioned above the penis, dramatically increasing visible length.
This procedure is also performed on men with buried penis due to prominent pubic fat and skin.

Penis lengthening through scrotal skin reduction
- Scrotoplasty -
Scrotal reduction or scrotoplasty involves separating scrotal skin from the penile body and creating a new angle between the penis and scrotum.

The procedure lasts about 45 minutes.

Penis lengthening and widening
by enlarging deep penile structures
- Corporoplasty -
This is true functional (not just cosmetic) penis enlargement. Through radical reconstructive surgery using various implants, the internal deep structures of the penis are enlarged. This procedure is only performed on patients with Peyronie's disease or erectile dysfunction.
Penis girth enlargement with autologous fat
- Lipofilling -
Fat is harvested via liposuction, typically from the pubic region (which can additionally increase length). The fat is prepared through decantation (sedimentation for higher fat concentration) with PRP Matrix addition. Using special blunt needles (cannulas), the fat is injected under the penile skin. Post-operation, uneven fat distribution and contour irregularities may occur. About 50% of the fat gets reabsorbed within the first few months.

Penis girth enlargement with autologous fat
- Lipcube Microfat System -
The difference from the outdated lipofilling method is using processed fat via the Lipocube Microfat System. Special cannulas are used for liposuction to obtain more uniform fat tissue, which is then centrifuged and micronized to create a paste-like product completely different from traditional lipofilling. This preparation provides more even contouring and significantly less fat resorption. It's more expensive than lipofilling due to specialized materials.
