How do shock waves work?
Extracorporeal shock wave therapy (ESWT) works by passing intense but short ultrasound shock waves through the target tissue. They were originally used to treat kidney stones without the need for surgery (lithotripsy - ESWL).
Today, Low-intensity Shock Wave Therapy (LSWT) is widely used for the treatment of all types of soft tissue injuries - tendinopathy, bone fractures, diabetic foot, sports injuries.
Treatment with low-intensity shock waves initiates an inflammatory response in the tissue where the shock waves were applied. The body responds by increasing blood circulation and metabolism in the area of impact, which in turn accelerates its own healing processes.
In the specific case of erectile dysfunction, shock waves improve potency through two mechanisms:
They activate angiogenesis (development of new blood vessels) inside the penis, which means that the blood flow in the penis increases and thus stronger erections are achieved; and
They restore the function of smooth muscles in the arteries of the penis - in most patients suffering from impotence, the smooth muscles in the arteries of the penis are damaged, which prevents the functioning of the so-called veno-occlusive mechanism responsible for the erection process.
What is plasma enriched with activated platelets - PRP? How does PRP Matrix with Hyaluronic Acid - HA work?
PRP is used safely with great success in many disciplines, including orthopedics, maxillofacial surgery, plastic and reconstructive surgery. In the field of genital regenerative medicine, PRP immediately found its place.
PRP is obtained directly from your blood. After a special processing procedure, the prepared blood plasma has a much higher concentration of activated platelets than the initial one. Platelet activation orchestrates the release of several potent cell growth and regeneration factors (FGF, PDGF, TGF-ß, EGF, VEGF and IGF). Platelet growth factors are signals that regulate cell growth and division, as well as the synthesis of extracellular tissue regeneration factors.
To achieve a longer effect inside the penis, PRP is used in the form of PRP-Fibrin Matrix (PRP-FM).
Another modification to improve results is the addition of hyaluronic acid (HA) to the PRP-FM solution. HA acts as a biological scaffold for regenerative processes, thus multiplying the effect of PRP.
How are vacuum devices used to treat impotence?
Vacuum erectile devices (VEP - vacuum erection pumps), with proper application (without the use of a clamping ring), increase blood oxygenation in the entire penis. The erection caused by the vacuum device facilitates tissue oxygenation, reduces fibrosis within the vascular tissue of the penis in the absence of nocturnal erection, and increases erectile function. Because of its ability to draw blood into the penis regardless of nerve and vascular disorders, VED has become a central part of penile rehabilitation protocols.

Who are candidates for LSWT+PRP Matrix therapy?

Men who do not tolerate the side effects of erection pills.
Men who do not have a good enough erection after taking the pill, regardless of the dose.
Men suffering from vascular impotence (disease of blood vessels of the penis).
A full diagnostic evaluation is always advised before treatment.
Before starting the therapy for impotence, it is necessary to perform an intra-cavernous test with prostaglandin PGE1 along with an ultrasound color-Doppler examination of the penis. Men who do not respond to the intra-cavernosal test are NOT good candidates for virility recovery therapy because their erectile mechanism is permanently damaged and they are unlikely to benefit from this treatment.
What are the expected results?
Combined therapy with shock waves, PRP Matrix and vacuum devices for ED helps in the following ways:
Patients with mild to moderate ED who respond well to PDE-5 inhibitors (Viagra, Kamagra, Cialis) can regain their natural erection and no longer need medication.
Patients with moderate to severe impotence who do not respond to medications and rely on injections, may begin to feel the effects of PDE-5 medications after shock wave therapy.

What should I expect during a treatment session?
Each treatment session lasts about 45 minutes.
Most patients feel slight discomfort during the session, but the whole procedure is not painful. It is important that if you feel uncomfortable, inform your doctor who will reduce the intensity of the shock waves.
Is the treatment painful?
Sometimes the treatment is a little uncomfortable at first, but not painful. Mild discomfort during the treatment illustrates that the shock waves have a positive effect.
Will there be any pain after the treatment?
Usually, patients feel very little pain or discomfort immediately after the treatment, but mild and diffuse pain may appear several hours later. This dull pain can last for a day and can be easily solved by taking painkillers (Ibuprofen). Do not use ice on the treated area as it can interfere with the healing process.
What is the best protocol for conservative treatment of impotence?
Our experience and previous studies show that there is no universal protocol that suits all patients. The number of sessions and the total duration of therapy depends on the severity of erectile dysfunction, the comorbidities present and the response to treatment. In our clinic, we recommend 4 or 5 sessions over a period of no less than 4 weeks.
Are the results permanent?
The biological effects of the therapy are permanent and have been established at a proven scientific level. However, erectile dysfunction largely depends on lifestyle and is usually a symptom of other health problems that lead to impotence. If a serious change in daily habits is not taken or the cause of impotence is resolved, there will be no lasting results.
What if the REGENERATIVE PROTOCOL® doesn't work?
Over 90% of men are very satisfied with this treatment, but it may take several months before the maximum effect is achieved.
